As a practice we appreciate that prompt and timely access is important when confronted with acute illness and worry. We also appreciate continuity of care when patients are dealing with chronic illness or are in the palliative stage of their illness.
A lot of planning, monitoring and communication go into balancing your health care needs and how various health care professionals within our Multi-disciplinary team can support you. Some people were maybe used to see their GP for every illness, minor or major, but the model of delivering primary care is changing to make it sustainable for the future. New health care professionals like pharmacists and physiotherapist are now an integrated part of our team as are our Nurse practioners, practice nurses, treatment room nurses, district nurses, health care assistants, social workers and health visitors. Our receptionists are trained and very competent in signposting you to the right healthcare professional and to do so they will ask you a few standard questions.
